How to submit your nomination

1. Read the Nomination Guide - please download at Nomination information and review. This contains all the information you will need to submit your nomination, including step-by-step instructions, the Awards categories, criteria and the referee form.
2. Complete the referee form - this form must be completed by someone other than the nominator and/or nominee and uploaded as part of the nomination.
3. Set up your account - if this is your first time using a nomination portal, click the Create new account button below and follow the instructions. This will allow you to begin your nomination submission. If you have forgotten your password, please contact veterans.employment.awards@dva.gov.au. Please note which nomination portal you are attempting to access in your email.
